After a decade of health policy, health system, and quality improvement focus on hospital readmissions, there has been little change in patient outcomes following hospitalization. During this seminar, Dr. Anderson will discuss current gaps in post-hospital care delivery and the potential to reimagine and redesign post-hospital care as the domain of population health and primary care.
