Age-Friendly care is an evidence-based framework to improve and personalize care of older adults. This talk will describe a multi-stage process for implementing Age-Friendly initiatives within an urban environment. The design and early impact of a delirium screening and intervention program and a collaborative mobility initative will be discussed.
